Job Title: Registered Nurse - Case Manager Job Specialty: Care Navigation Job Duration: 8 weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: 40 Experience: Minimum of 1 year License: Valid state license; pending license accepted Certifications: BCLS preferred Must-Have: - Epic and CarePort experience- Acute Hospital Work Setting experience- Required Case Management/Utilization Review Skills such as care coordination, discharge planning, needs assessment/orders for DME, and prior authorizations Job Description: - Focus on discharging patients, with ratios of 1:20-24 during the week and 1:80 on weekends.- Engage in floating between facilities as needed to meet facility demands.- Work with adult and geriatric patients with a wide range of diagnoses, including trauma, cardiovascular, renal, stroke, post-surgical, respiratory, oncology, CHF, and vascular issues.- Operate in a teaching hospital with residency programs, catering to Level 1 Trauma, Level 1 Neuro, and various other units.- Utilize special equipment, including laptops, for effective patient care management.- Employ skills in care navigation/discharge planning with charge experience in a teaching or community hospital setting.- Participate in a team environment with expectations to complete a basic Dysrhythmia/EKG Competency Exam for various units.- Mandatory COVID-19 and flu vaccinations, with exemptions available for medical or religious reasons.- Engage in a professional setting with business casual attire and the option to wear a white lab cover jacket.- Mandatory completion of modules on the first day of orientation.- Weekend work is required every other weekend.- Handle the wide range of responsibilities within a Level I trauma center.
