I. JOB SUMMARYSocial Worker is responsible for addressing psychosocial, financial and discharge planning needs of patients in the ARU/SNU. This includes, but is not limited to, Advance Directives issues, patient/family advocacy and coordination of community resources

II. ORGANIZATIONAL SUMMARY

III. ESSENTIAL FUNCTIONSA. Participates in discharge planning, development and implementation of social care plans and resident assessments. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary.B. Participates in interdisciplinary team meetings.C. Promotes optimum transition of patient for appropriate and safe placement for discharge. Provides ongoing communication with patient and responsible persons of progress/status during stay.D. Provides information to the patient/family on Medicare/Medicaid as well as other financial assistance programs available. Provide consultation to staff in efforts to solve patient/family concerns related to psychosocial concerns, discharge planning and placement after SNU/ARU.E. Demonstrates understanding of rules and regulations that govern SNU and ARU.F. Demonstrates autonomy, professional judgement, responsibility and reliability.G. Provide written and/or oral reports regarding social service programs and activities as directed by such committee(s). Conduct grievance tracking and report out in the appropriate meeting. Evaluate and implement recommendations as they may pertain to social services.H. Participates in the quality programs for SNU/ARU.I. Demonstrates flexibility to meet patient/family needs.J. Maintains required department confidentiality and abides by HIPPA regulations.K. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.L. Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development and participates in medical center and/or departmental education programs.M. Communicates with and performs duties following the philosophy of MercyOne North Iowa Medical Center and the Sisters of Mercy reflecting the organization's Mission, Values, and Impact our key results.N. Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.O. IV. ADDITIONAL FUNCTIONSA. Conforms to the NASW Code of Ethics.

V. POSITION QUALIFICATIONSA. Education:1. Bachelor's degree in social work from a CSWE accredited program or bachelor's degree in a human services field including, but not limited to, sociology, special education, rehabilitation, counseling and psychology.2. Master's degree in social work desirable.B. Experience1. One year professional social work experience in long term care preferred.2. One year of supervised social work experience in a health care setting working directly with individuals required.C. Special Skills, Competencies, and Aptitudes1. Knowledge of State regulations and guidelines related to long term care and specifically regarding social worker responsibilities (ie. MDS, dischargeplanning, Advance Directives)2.Excellent knowledge and practice of psychosocial assessment skills, including social and emotional factors, counseling for long-range health planning and decision-making, community resource planning and/or short-term therapy, etc.3. Computer skills, typing proficiency, including Word, Excel4. Ability to work under time pressure5. Able to set and organize own work priorities and adapt to changing care settings and priorities using a high degree of initiative and judgment.

Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.X Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.Completes Advanced Cardiac Life Support (ACLS) - American Heart Association within 6 months of hire and maintains current status.Completes Pediatric Advance Life Support (PALS) - American Heart Association within 6 months of hire and maintains current status.Completes Neonatal Resuscitation Program (NRP) - American Heart Association within 6 months of hire and maintains current status.Completes MANDT Training within 6 months of hire and maintains current status.Completes Trauma Nurse Core Course (TNCC) within 6 months of hire and maintains current status.
