Provide exceptional nursing care to residents in the subacute unit, focusing on rehabilitation and recovery.Monitor and assess residents’ conditions, administering medications and treatments as prescribed.Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.Perform regular assessments and adjust care plans based on residents’ progress and needs.Educate residents and their families about care plans, treatments, and health management.Ensure adherence to all regulatory requirements and facility policies.Support the overall well-being of residents by fostering a safe and nurturing environment.
Subacute Nurse
High School Diploma or equivalent.Previous LTC/SNF experience preferredRN certification in NJSerious Applicants
