A medical services organization in New York City is looking to add a licensed Registered Nurse (RN) to one of their facilities in the Bronx. In this role, the Registered Nurse (RN) will be responsible for developing plan of care and coordinating the delivery of healthcare services for persons receiving services in an assigned program unit.

The Registered Nurse (RN) *Coordinate the delivery of routine and specialty healthcare services to address assessed needs of persons receiving services in assigned program unitAssess and establish healthcare outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicatedDevelop plan of nursing services and oversee the implementation of the health care plan by educating persons receiving services, DSP/Caregivers and families in the plan of nursing servicesEducate, monitor and supervise staff in carrying out ordered treatment plan related to health needsEvaluate the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessaryAct as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providersComplete and maintain all required written documentation on the approved electronic health record systemConduct general and individual specific health related trainings for persons receiving services, direct care staff, and other team members, as neededComplete relevant medical documents, MIPS and other required consent packets, surrogate packets, writing incident reports, death reports, assisting in developing policies, procedures and forms; and establishing working systems.Perform all other duties, as assigned.*Graduate of an accredited School of NursingNYS Registered Nurse (RN) licenseSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired *

*Bachelors Degree in NursingPrior Nursing experience and/or experience working with persons with Developmental Disabilities
