Lake City, FL 32024The RN Case Manager is responsible for the planning and management of an assigned caseload of patients across the Lake City/Columbia County area. As a member of an assigned care team, the RNCM coordinates resources and performs a wide range of skilled nursing activities for an assigned caseload of patients.Position DetailsWork Locations: Lake City/Columbia County area; home health, LTC and ALFsCompensation: Base salary $68,000 - $71,000 ; full benefits package including 23 days of PTO annualPrimary Responsibilities S/he is responsible for ongoing assessment of patient/caregiver needs, symptom management, teaching of appropriate care related to disease process, and participating as a member of the interdisciplinary group (IDG) in providing end-of-life care/support for patients, caregivers and the community.Serve diverse area, providing hospice and palliative nursing care management. Flexibility to provide community education, provider education, advanced care planning, admissions and oversite of patients in 3A area. Flexibility to work within any region of Community Hospice and Palliative care as needed.Provides information about Community Hospice services and philosophy to patients, caregivers, physicians, LTC administrators and directors of nursing, and the community.Ability to implement Medicare Local Coverage Determination to assess patients for Hospice eligibility.Responsible for education and mentoring of new staff members.Builds, develops and maintains relationships with referring physicians, area social workers, discharge planners and community agency staff in order to network and facilitate admission and placement of patients.Educates medical community on what are hospice appropriate referralsEducates medical community on the patient/family benefits of early referralsCultivates, develops and enhances existing relationships with physicians, hospital, nursing homes and assisted living facilitiesAssists in providing in-service programs or public relations activities. Participates in hospital committees, speaks at hospital end-of-life in services, coordinates staff education with Hospice Representatives and actively explores new opportunities for growth of patient referrals and admissionsAvailable to provide community education as requested. Collaboration and coordination of care with community providers related to hospice and palliative care services.All employees assigned this job title must be willing and able to flex their work location within and between any Community Hospice & Palliative Care locations, including home health, ALF and LTC locations. Education/Experience:Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:Possession of a degree from an accredited school of nursing program, 2 (two) year Associate Degree, or a Bachelor's Degree in Nursing.One (1) year Registered Nurse experience with one (1) year hospital medical-surgical/oncology experience preferred orFive (5) years Licensed Practical Nurse experience within the past five (5) years, with one (1) year hospital medical-surgical/oncology experience preferred.Knowledge of appropriate educational principles and methods as applied to hospice nursing.Working long and occasionally irregular hours.We are an equal opportunity employer.We do not discriminate on the basis of race, color, religion, marital status, age, national origin, disability, pregnancy, genetic information, gender, sexual orientation, veteran status, or any other status protected under federal, state, or local law.
