Check Us Out Online www.nychealthandhospitals.org careers.nychhc.org Docs4nyc.org Job Description Under general supervision and in accordance with New York State requirements for written practice protocols and written agreements or for collaborative relationships with qualified physician(s) or licensed health care facility serves as a licensed, independent practitioner responsible for planning, coordinating, delivering and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health Hospitals policies and procedures, state and federal rules and regulations and nursing standards of care. Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families. Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures. Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physicians intervention. Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary. Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions. Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals. Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention and health promotion, records and maintains a system of relevant documentation. Participates in internal/external quality assurance/performance improvement (QA/PI) activities, programs, and training, as required. Performs other related duties, as directed. Minimum Qualifications For Assignment to Nurse Practitioner A valid New York State license and current registration to practice as a Registered Professional Nurse; and Current certificate as a Nurse Practitioner issued by the New York State Department of Education (NYSED); and A current certification issued by the State Education Department to prescribe medication.
