OVERVIEWLightHouse seeks a blind-positive Intake Coordinator, who will be LightHouse student's first connection to potential blind services throughout the Agency. This person will have a high social emotional intelligence and will be able to meet people where they are.
QUALIFICATIONS:
At least 2 years' experience in a community-centered setting
Must be an effective user of Microsoft Office Suite, including Outlook, Word, and Excel. Salesforce or other database experience, and knowledge of Zoom or other web conferencing platforms.
Must comply with HIPAA regulations in all communications and handling of documents.
Experience working with Medicare, Medi-Cal or other third-party billing entities and their requirements for documentation is preferred.
Experience working with the blind/disabled community strongly preferred.
A preference of an associate degree in a social science field, professional nursing degree, or equivalent
Training and/or formal understanding of health-related terminology and understanding of health conditions.
Ability to handle sensitive information with conscientiousness and respect.
A high level of social-emotional intelligence, demonstrating patience and empathy.
Strong coaching skills, including positive problem solving and professional communication.
Ability to multi-task and work independently
Bi-lingual/bi-cultural English/Spanish is highly desired.
Maintain up-to-date First Aid and CPR certifications, to be paid for by LightHouse.
JOB RESPONSIBILITIES:
Meet with students and their support networks, as applicable, to collect all necessary documents to initiate comprehensive services.
Gathers all necessary programmatic and/or patient specific information and documentation.
Works with department directors and program managers to ensure students have been contacted within a timely manner and document status of student participation.
Accurately inputs all student data into Salesforce and any relevant Enhanced Health Record systems (EHR) in a timely manner.
Ensure all students have the requisite paperwork to begin receiving services from LightHouse. This includes both external paperwork and authorizations from the referring doctor, Department of Rehabilitation counselors, and Veterans Affairs Counselors, and internal paperwork like the LightHouse Student Waivers.
Is the primary point of contact with DOR counselors for authorization procurement.
Support rehabilitation program staff to ensure students receive all necessary/required equipment.
Produce monthly service reports tied to specific payment sources.
Work collaboratively with the Finance Team to ensure accurate billing and payment reconciliation.
Respond to all student inquiries in a timely manner.
Regularly meet with other Connections Department colleagues and other Intake Coordinators to ensure consistency of LightHouse student intake process across all LightHouse service areas.
Collaborates with Service Navigators in the coordination of both internal and external service authorizations.
Ensure entry of all student service data into Salesforce and other relevant systems within 5 days of service date.
Submit monthly reporting, billing, and reimbursements accurately and timely by the 5th business day of preceding month.
Willingness to perform other related duties as assigned.
PHYSICAL REQUIREMENTS:
Ability to carry or transport 20lbs
Sit/Stand at a desk and perform intensive computer work, as needed
Operate standard office equipment
Must be able to independently travel to all LightHouse physical locations and events as needed.
