*Will accept* RN Discharge Planner or Licensed Social WorkerSocial worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already
*Certs Required*: BLS,RN or LMSW/LGSW/LCSW
*Work Setting skills Req*: Cerner, Acute Hospital, Insurance Company, Long Term Acute Care/Rehab/Skilled NursingWork Setting skills Preferred: Home Health, Hospice, telephonic, previous charge experience
*Case Management/Utilization Review skills required*: Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment/ Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker’s comp case management
Regulatory Skills Required: CMS: Centers for Medicare and Medicaid Services, Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), The Joint Commission/ Core Measure/National Safety GoalsRegulatory Skills Preferred: Worker’s Compensation
Any Special Requirements: Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters
Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU,
Holidays – as neededWeekends – Every 5-6 weekends must work one shift)Cerner is requiredUniform: Business Casual-Associate’s appearance should be clean, neat and professional. NO SCRUBSDoes not accept 1st timers

Pay: $55.00 - $65.00 per hour
