Become the Case Manager You've Always Wanted to Be
Collaborate with an interdisciplinary team to develop treatment plans aligned with patient needs.
Establish tentative discharge plans and prepare contingency strategies.
Engage actively in planning and executing a seamless patient discharge experience.
Monitor the patient experience and ensure quality, service timeliness, and appropriateness.
Lead weekly team conferences to discuss treatment plan modifications.
Maintain all necessary documentation including case management addendums.
Stay informed on regulations, company policies, and operational procedures.
Review case management reports and implement necessary actions.
Attend Acute Care Transfer meetings, identifying trends and collaboratively reducing transfers.
Meet with patients and families within 24 hours of admission to establish communication.
Perform assessments to set goals and complete necessary documentation promptly.
Educate patients and families about rehabilitation processes and your role as a Case Manager.
Schedule and conduct family conferences as required.
Assist patients in securing resources to prevent discharge delays.
Ensure compliance with orthotic and prosthetic regulations.
Make timely referrals and ensure accurate post-discharge documentation.
Participate in utilization reviews and facilitate trend analysis.
Be part of the on-call case management schedule when necessary.
Qualifications
Licensure and Certification:
Must have qualifications to independently perform assessments in your discipline (RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required, you must hold a current active license.
Must be eligible for CCM® or ACM certification upon entering this position or within two years.
CCM® or ACM certification is required or to be obtained within two years of placement in this position.
Minimum Qualifications:
For Nursing, an Associate Degree in Nursing with RN licensure required; BSN preferred. A diploma is acceptable only if it meets state licensure requirements.
For all other eligible licensed or certified healthcare professionals, a minimum of a bachelor's degree is required; a graduate degree is preferred.
A minimum of two years of rehabilitation experience is preferred.
