Job Details
The advanced practice provider (Nurse Practitioner/Physician Assistant) within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APP provides comprehensive care at the Sidney Kimmel Cancer Center outpatient practice in Center City, PA.
Responsibilities:
- Provide individualized patient care, teach, and counsel patients.
- Work collaboratively with the care team.
- Perform complete history and physicals and document in the medical record.
- Order and collect data using appropriate assessment techniques and relevant supporting diagnostic information.
- Serve as the first responder for nursing and house staff in the event of patient status changes.
- Assess risks associated with the care of acute and complex chronically ill patients.
- Collaborate with the interdisciplinary health care team in making diagnoses of acute and complex chronic conditions.
- Manage diagnostic tests through ordering, interpretation, performance, and supervision.
- Formulate and document a plan of care to address complex acute and chronic health care needs.
- Implement and modify the plan of care.
- Document key components of patient progress via daily progress notes and summaries.
- Maintain ongoing communication and collaboration with the interdisciplinary health care team.
- Demonstrate professional practice behaviors including preceptor/mentoring and education of graduate students.
Qualifications:
- Masters Degree.
- National Board Certification as a Primary/Family Care Nurse Practitioner.
- BLS certification.
