We are seeking a dedicated and experienced Assessment Nurse to join our team at HomeWell Care Services IL250 in Glendale Heights, IL. As an independent contractor, you will play a crucial role in assessing the needs of our clients and developing care plans to ensure their well-being. This is a per visit pay job. Responsibilities: Conduct comprehensive assessments of clients' physical, emotional, and mental health Develop individualized care plans based on assessment findings Coordinate with other healthcare professionals to ensure holistic care for clients Provide education and support to clients and their families Document assessment findings and care plans accurately and timely Requirements: Valid RN license in the state of Illinois Minimum of 2 years of experience in home health or a similar setting Strong communication and interpersonal skills Ability to work independently and collaboratively within a team Compassionate and empathetic demeanor About Us: HomeWell Care Services IL250 has been providing exceptional home care services in Glendale Heights, IL for over 10 years.
