Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration. Attestations not allowed. Needs to be rerun every 2 years with RN license renewal & upon expiration. Temporary Licenses are NOT accepted – must have a valid permanent license. Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required. Positive MMR titers ONLY from within the last 15 years. If low titer was completed within 30 days prior to start, only 1 booster is required to start and 2nd booster must be given 30 days after 1st dose.
