Job Responsibilities

• Deliver high-quality nursing care in post-acute care facilities
• Observe residents' conditions and update the nursing team
• Monitor and report on residents' vital signs
• Maintain a clean and safe environment by performing sanitation and changing linens
• Assist residents with personal hygiene, feeding, grooming, and daily living activities
• Reposition and transport residents as needed within the facility
• Communicate effectively between staff, residents, and families for ongoing support
• Engage regularly with residents and respond to their needs
• Adhere to care plans and comply with all applicable laws and regulations

Qualifications and Skills

• Current Connecticut Certified Nursing Assistant (CNA) certification
• Able to lift at least 50 lbs
• Familiarity with medical terminology and nursing care procedures
• Strong attention to detail
• Effective communication and interpersonal skills
