Clinical Informatics Specialist
is responsible for participating in the planning, development, training, implementation, evaluation and maintenance of Concordia Home Care Home Base (HCHB) Electronic Medical Records (EMR). The Clinical Informatics Specialist will demonstrate an in-depth knowledge of the EMR systems, streamlining processes and procedures, analyze changes and enhancements to application and determine their impact on existing systems. The

Associate or bachelor’s Degree recommended or experience 1 year of experience in Hospice or Home Health recommended. Experience with software systems in a clinical environment. Experience with project management, recommended. Excellent organizational and communication skills: both oral and written Excellent interpersonal and customer service skills with the ability to deal effectively with a wide variety of users with computer knowledge and skills. Ability to self-motivate and work with a team (collaborative environment) Reliable, independent means of transportation. Driver must have valid driver’s license with vehicle insurance at the limits established by state law DUTIES AND RESPONSIBILITIES for the Clinical Informatics Specialist: Creates, develops, and optimizes workflows, electronic forms, new build opportunities, and system configurations. Creates, develops and analyzes reports and data analysis Assists with onboarding with alignment of EMR workflows Monitors documentation for Quality Assurance Supports the integration of systems with hospitals, healthcare providers, and third-party software solutions Supports clinical processes translating to improved quality metrics. Documents key processes, tip sheets, and educational materials necessary to support software, hardware, and related systems Constantly reviews current processes to improve performance and increase efficiencies. Maintain excellent customer service to employees, vendors, patients and physicians Supports staff and management in decision making and implementation of new processes and systems. Notifies software vendors of issues, creates and responds to service order tickets and continue to follow up on issues until resolved. Works with the Education Team to collaborate with any operation/educational/informatics initiative. Assists in working with software vendors, planning, design, testing, training implementation, post go live support and software updates.
