$105,000 - $120,000 Job Brief: We are a leading non-profit provider of residential, day habilitation, Article 16, vocational, wheelchair, and transport services throughout New York City and the Hudson Valley to people with intellectual and developmental disabilities. It is a time of growth at Constructive Partnerships (CP) Unlimited! The Agency recently expanded its constituency to include homeless services. Responsibilities: Provide onsite comprehensive day-to-day leadership and oversight for the ESD Program, integrating the Critical Time Intervention (CTI) and Transitional Residential Services (TRS) components. Lead, manage, and evaluate the day-to-day operations of multiple locations, including hospital referral sites and transitional residence in Yonkers and the Hudson Valley area. Oversee the implementation of the CTI phased model, ensuring precision in the care transitions from hospital settings to the community. Ensure that the TRS component adheres to trauma-informed care and person-centered approaches, fostering an environment of safety and emotional regulation for residents. Serve as the liaison with external stakeholders, including hospitals, OPWDD service providers, and regulatory agencies, building strategic partnerships. Lead and support quality improvement initiatives, monitoring program performance, client outcomes, and compliance with state and federal regulations. Foster a culture of collaboration, accountability, and recovery-focused care among all ESD Program staff. Requirements: A minimum of 5 years’ demonstrated experience leading and managing a team in the behavioral health field, focusing on populations with co-occurring mental health and I/DD needs. A master’s degree in human resources or related field. Must be a Licensed Clinical Social Worker (LCSW) in NYS. Extensive knowledge of trauma-informed care, care management models, and state/federal regulatory standards. Exceptional written and verbal communication skills coupled with organizational skills required.
