About the Facility
The Colorado State Veterans Community Living Center at Rifle is located in Colorado's scenic western slope, offering a mild climate and beautiful views. It provides 89 beds of skilled nursing care, including an 18-bed Special Care Unit for residents with Alzheimer's or related conditions. The center serves veterans, their spouses, and eligible "Gold Star" parents, emphasizing compassionate, holistic care.
Job Description
Provides basic, hands-on patient/resident care in a safe, infection-controlled environment. Responsibilities include bathing, dressing, grooming, hygiene, skin care, range of motion, positioning, ambulating, transferring, toileting, feeding, transporting, basic assessments (vital signs, blood pressure, weights), and charting/documentation.
Primary Job Duties
- Daily Nursing Care: Assists with bathing, feeding, dressing, transferring, ambulating, positioning, lifting, skin care, and social interactions. Supports resident safety and mobility programs, including use of assistive devices and performing CPR as needed.
- Safety: Ensures resident and staff safety through proper techniques, environment monitoring, and use of safety equipment. Performs basic assessments and maintains infection control practices.
- Communication: Collaborates with team members to ensure proper resident care, reports changes in condition, and maintains a therapeutic environment.
- Transport: Transports residents for medical appointments, attends training, and ensures effective shift communication.
Minimum Qualifications
- High school diploma or GED plus at least 1 year of CNA experience, or
- Valid CNA certification from Colorado or a nurse licensure (LPN) from Colorado or a Nurse Licensure Compact state.
Conditions of Employment
- Background check, TB test, valid driver’s license, current CNA license, CPR certification, and ability to work shifts including holidays and weekends.
- Must be at least 18 years old and able to pass all employment requirements.
