Are you a highly skilled and motivated Registered Nurse (RN) looking for a rewarding opportunity in the healthcare industry? Do you have a passion for providing compassionate care to patients and their families? If so, we have an immediate opening for a Hospice Registered Nurse at Emblem Hospice Tucson.
Emblem Hospice Tucson offers life-changing service to all our patients while also providing the emotional support and hospice education for the patient, their family, and/or caregivers. Our dynamic team of nurses, aides, social services, and spiritual care professionals, along with our compassionate group of volunteers and experienced management, work together to provide comfort where there is pain, solutions where there are challenges, and peace where there is chaos in the lives of our patients and their loved ones.
Section 1: Duties & Responsibilities Completes comprehensive assessments of patients and families to determine hospice needs, including physical assessments and history of illness. Provides professional nursing care by utilizing all elements of the nursing process. Develops care plans based on nursing diagnosis and incorporates palliative nursing actions. Administers medications and treatments as prescribed by the physician. Assists patients with activities of daily living and facilitates their efforts toward self-sufficiency and comfort care. Acts as a Case Manager and coordinates patient care for assigned caseload. Section 2: Communication
Maintains accurate and relevant clinical notes regarding patient's condition and care given. Communicates with physicians and community health-related persons to coordinate the care plan. Teaches patients and families self-care techniques and provides medication and diet instructions. Provides a safe environment for patients and assists in providing continuity of care. Works with the interdisciplinary group to meet the emotional needs of patients and families. Attends interdisciplinary group meetings. Section 3: Additional Duties
Participates in on-call duties as defined by the on-call policy. Ensures necessary equipment and services are available. Supervises ancillary personnel and delegates responsibilities when required. Maintains and upgrades professional knowledge and practice skills through continuing education. Actively participates in quality assessment performance improvement teams and activities.
