We are seeking a dedicated and experienced RN (Registered Nurse) to join our hospice team as a full-time Facility-Based Hospice Case Manager, which includes a $10,000 signing bonus. The schedule is Friday to Tuesday.
This role involves planning, organizing, and directing care for hospice patients at Kline Gallands Skilled Nursing Facility in South Seattle. The RN Case Manager should have experience in nursing, with an emphasis on community health education and experience. The professional nurse collaborates, coordinates, plans, and directs services to meet the needs of individuals and families within their homes and communities.

Essential Job Functions/Responsibilities
- Completes new patient admissions, initial assessments, and ongoing comprehensive assessments of patients and families to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses.
- Supports symptom and care management for all hospice patients at Kline Galland Home skilled nursing facility as scheduled.
- Collaborates with facility staff and leadership to support hospice patients within regulatory requirements.
- Attends regular hospice team meetings and clinical rounds for patient review at the skilled nursing facility.
- Provides professional nursing care utilizing all elements of the nursing process.
- Must hold a current Registered Nurse (RN) license in WA or possess a multistate license.
- Minimum of 1 year of experience in hospice or palliative care.
- Strong clinical assessment and care planning skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.
- Proficiency in electronic medical records (EMR) systems.
