Position Overview
The Illinois Department of Public Health is seeking a highly motivated individual to perform a variety of difficult and responsible clerical functions for the Licensure Program. This individual exercises some choice in procedures and acts independently in disposition of routine matters or in personal contact with others to give non-technical information.
Essential Functions
- Prepares, microfilms, verifies and inserts all licensure, application, plan review, supplemental information and correspondence for the Licensure Program.
- Provides assistance in reviewing resident room listings.
- Maintains listing of long-term care administrators, sends follow-up letters and provides assistance for reviewing forms submitted for changes of administrator.
- Maintains working files for all program materials including Health Facilities Planning Board correspondence, licenses, applications, floor plans, room listings, supplemental information, correspondence, plans, drawings and requests for adult day care and out-patient therapy programs for all proposed and current long term care facilities.
- Purges all files on a continual basis, updates survey files, prepares and send files for storage to storage facility.
- Reviews the status of all entities operating long term care and community living facilities to ensure entities designed as the operator/licensee are in good standing.
- Requests updates of provider association affiliation on an annual basis.
- Orders, receives, and distributes printouts of projects in the construction data base to the appropriate staff for review.
- Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
- Requires knowledge, skill and mental development equivalent to completion of high school.
- Requires one (1) year of related office experience.
- Requires ability to keyboard accurately at 35 wpm.

Work Hours: Monday-Friday 8:30AM-5:00PM
