A nonprofit organization in New York City is actively seeking a Licensed Practical Nurse (LPN) to join their growing Primary Care team at one of their shelters in Brooklyn. About the Opportunity: Specialty: Primary Care Setting: Homeless Shelter Schedule: Monday to Friday Hours: 8:30am to 4pm (flexible for 9am to 4:30pm) Patient Population: Homeless Patient Volume: 20-25 patients a day Responsibilities: The Licensed Practical Nurse (LPN) Provide nursing screening of all new clients Administer morning and afternoon medications Administer decanoate injections according to orders of psychiatric provider Participate in weekly clinical meetings. Assist in triaging psychiatric, medical and substance abuse problems to appropriate providers Communicate with onsite & off-site psychiatric providers, as necessary and appropriate Provide ongoing health education and support to clients and medication teaching, as needed Maintain documentation in compliance with agency policies and procedures Perform other duties, as needed Qualifications: Bachelor's Degree NYS Licensed Practical Nurse (LPN) Computer savvy Microsoft Office proficient Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills: Experience working with Homeless individuals
