A nonprofit organization in New York City is actively seeking a Licensed Practical Nurse (LPN) to join their growing Primary Care team at one of their shelters in Brooklyn.

The Licensed Practical Nurse (LPN)
Provide nursing screening of all new clientsAdminister morning and afternoon medicationsAdminister decanoate injections according to orders of psychiatric providerParticipate in weekly clinical meetings.Assist in triaging psychiatric, medical and substance abuse problems to appropriate providersCommunicate with onsite & off-site psychiatric providers, as necessary and appropriateProvide ongoing health education and support to clients and medication teaching, as neededMaintain documentation in compliance with agency policies and proceduresPerform other duties, as neededQualifications:
Bachelor's DegreeNYS Licensed Practical Nurse (LPN)Computer savvyMicrosoft Office proficientGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:
