Magnolia Health Systems is seeking a Social Services Coordinator to become a part of our team where our primary goal is to provide exceptional care to our residents. About the Role: Magnolia Health Systems is seeking a Social Services Coordinator to
- Assist in the entire admissions process from pre-admissions (pre-screening, tours, etc.) to discharge (placement arrangements, homecare referrals, etc.)
- Provide social services in accordance with accepted facility, professional and governmental regulatory standards.
- Act as a resident advocate and liaison with facility staff and with community resources as necessary.
- Develop, as neede d and participate in the implementation of an appropriate
- B eha vior Management Program for applicable residents
- Indiana board certification in social work under IC 25-23.6-5-1
- 1+ years’ experience in health care setting working directly with individuals
- Bachelors or advanced degree in social work or a human services field
