Generous PTO, Medical, Dental, Life Insurance, etcCompetitive compensation with achievable incentive bonus programWhat you'll do as a Medical Social Worker (MSW) Case Manager: Conducting comprehensive psychosocial assessments of patients and their families to identify their needs, strengths, and resourcesDeveloping and implementing care plans that address the psychosocial needs of patients and their familiesProviding counseling and emotional support to patients and their families, including grief and loss counselingCoordinating community resources and services to meet the needs of patients and their familiesServing as an advocate for patients and their families, helping them to navigate the healthcare system and access needed resourcesParticipating in interdisciplinary team meetings to develop and review patient care plansMaintaining accurate and timely documentation of all services providedRequirements for Medical Social Worker (MSW) Case Manager: Master's degree in Social Work from an accredited school of social workCurrent state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)Minimum of 2 years of experience working in hospice or palliative careExcellent communication and interpersonal skillsAbility to work effectively as part of an interdisciplinary teamKnowledge of community resources and services that can support the needs of hospice patients and their familiesAbility to provide compassionate and empathetic care to patients and their families
