The Care Coordinator will be responsible for providing direct support and coordinating care services for patients. The ideal candidate will have experience in direct patient care, or related education and possess strong organizational and communication skills with an empathetic mindset.What You Will Be Doing As A Care Coordinator:Coordinate patient care activities, including scheduling appointments, arranging referrals, and ensuring follow-up careMaintain accurate and up-to-date patient records in electronic medical record systemsCommunicate with patients, healthcare providers, and insurance companies to ensure seamless care coordinationProvide individuals with physical, emotional, and behavioral support allowing them to actively participate in activities offered while encouraging personal choices and assisting in maintaining their personal safetyProvide transportation, in personal and company-owned vehicles, throughout the day to and from the individuals' homes and activity locations in the communityEducate patients on available resources and support servicesOur Ideal Care Coordinator Has:Customer service orientation-proactively assist clients in a professional, friendly manner to create the best possible experience by building relationships, understanding quality standards for service, and utilizing customer needs assessmentsSocial perceptiveness-be aware of other people's reactions and understand why they react the way they do.
