Supervisory Responsibilities · Responsible for coaching, training, onboarding, employee engagement, staff rounding, timekeeping, onboarding new staff and performance evaluations. Leads daily management huddle boards. · Delegates patient care tasks. Provides clinical supervision in area of specialty as required. · Actively participates in enhancing patient experience, quality control, and process improvement initiatives. Ensures regulatory compliance and assists with incident resolution, policies/procedures, and emergency preparedness. · Identifies opportunities for improvement, recommends process improvements utilizing operating model, and takes necessary steps to improve processes · Responsible for managing all DPH and Joint Commission audits. Makes sure all staff are compliant with education, all files are reviewed, responds to all inquiries.
Clinical Responsibilities: · Completes all documentation including but not limited to: evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary. · Participates in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care. Helps to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources as related to care transition planning.Qualifications · Physical Therapist (PT) or Occupational Therapist (OT) license in the state of CT · Bachelors, Masters or Doctoral Degree in Physical Therapy or Occupational Therapy · Minimum 5 years’ clinical experience required · Experience as a clinical instructor or mentor required · Strong verbal and written communication skills required · Computer proficiency in Microsoft Word, Excel, PowerPoint as well as electronic health records and other software applications · Ability to solve problems and work independently with minimal direction · Teamwork and mentoring skills · History of strong leadership, communication, and clinical skills
