$85,880 to $95,000 based on experience Sign-on Bonus:

M-F 8:30AM-5:00PM Visiting Client Territory:
Port Jefferson area. Would need to be present in each specific area when the Manager of Clinical Services is unavailable for that area.

Performs on-site supervisory and teaching visits to evaluate caregivers Provides clinical orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in-services for field staff. Performs initial nursing assessments Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs. Prepares clinical progress notes ensuring any changes in the patient's condition is reported Monitors clinical records for complete and accurate documentation.
