Hospice RN Case Manager / Registered Nurse
HealthCare Support is actively seeking a
Hospice RN Case Manager / Registered Nurse
to fill an opening with a
prestigious Hospice organization in Tennessee . Location - Murfreesboro area
Daily Responsibilities for
Hospice RN Case Manager / Registered Nurse
:
This RN Case Manager will visit hospice facilities to provide patient care and educate referral partners, including Nursing Home and LTC RNs. The RN Case manager will work alongside an NP, RN team, Social worker, and other case managers to collaborate and promote ongoing care Provided wound care, monitored vitals, administered medications, and performed death pronouncements as needed. Schedule: Monday - Friday, 8:00am - 5:00pm On-call one night per week from 5pm-11pm (no weekends)
Required Qualifications for
Hospice RN Case Manager / Registered Nurse
:
Active RN License in TN At least 2 years of RN experience Hospice/Palliative care experience Related experience in Hospice or Home Health or Oncology or ICU or Med/Surg or LTC/SNF Benefits for
Hospice RN Case Manager / Registered Nurse

Mileage reimbursement Full health benefits PTO - 6.77 hours accrued per pay period (increased after 3+ years of employment) - starts accruing day 1 Extended Sick Bank - 2.77 hours per pay period - starts accruing day 1 403b - company matches up to 3% of employee contribution at 50% rate Annual merit increases - up to 3% based on performance Annual bonus up to $3900/year
Pay Details: $74,173 - $84,886 per year (based on experience) + bonus eligibility
