Salary:
RN $45/hr. LPN $38/hr.
Status:
Days full time, 40 hours per week
Schedule:
Days, with Manager on Duty (MOD) weekend rotations Ask us about our Director of Nursing and AIT training programs.

A little more about our center:
Community Care Center is a large inner-city Skilled Nursing Facility. We have patients of all ages with multiple levels of care, including long term care and a large behavioral health (psych) population.
We have a new Dialysis program coming in Summer 2025 The average age of our patients is mid-40s Along with our medical/skilled patients, the most common diagnoses of the rest of our population is schizophrenia, bi-polar and major depressive disorders. The Staff and patients are welcoming and say hello as you walk through the hallways. We are located near W 43 ST & State Street, just off 90/94 and a few blocks from the train station. What you will do in this role:
The Restorative Nurse Coordinator (RN or LPN) is responsible for coordinating restorative care for our center. You will assist residents in attaining or maintaining the highest level of physical functioning possible. You will work closely with our therapy team to help patients reach their clinical goals and prevent declines.

What we need from you:
Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL 2+ years' experience in a SNF setting as a Restorative Nurse The restorative/rehabilitation certification is ideal, but not required Electronic medical records experience is helpful (Point Click Care or PCC is ideal)
