Full Time Case Manager
for our
Outpatient Department

, NJ . Our
Outpatient Program
uses a multi-disciplinary approach; emphasis is placed on individualized assessment, goal-oriented treatment planning, periodic case review, and continuity of care for all persons served. Job Description: Function as part of OPD team Interfaces with OPD psychiatrist Assists staff psychiatrist with scribing clinical assessments Completes treatment plans and maintains staff psychiatrist caseload of clients Completes documentation according to agency and DMHAS standars Job Requirements: Requires bachelor's degree in a human services field Visit our website www.ccannj.com CURRENT EMPLOYEES: Please be advised that current employees applying to open positions within the Agency are required to fill out an Agency Internal Job Application Form which is available for download to all employees on HR's public folders in Outlook.
