Encourages and facilitates cooperation within the organization and with customer groups; fosters commitment, team spirit, pride, trust.Education and Experience RequiredMust be licensed as a Physical Therapist in the state of Minnesota.Must obtain CPR certification within 6 months from date of hire.Must have a valid driver's license and use of an insured personal vehicle.Must complete and pass a state required background study.Able to communicate verbally and in writing to the extent required by the position.Able to physically perform the duties required by the position.Required to have the ability to read, write and speak English well enough to communicate with staff, clients and visitors.Key ResponsibilitiesDirect Client Care - Evaluate clients; obtain and interpret subjective and objective data. Develop treatment plans and programs including short-term and long-term goals in accordance with physician orders. Provide quality care that assists clients to meet goals; complete re-evaluations per requirements.Client Records and Billing - Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required. Complete and document supervisory visits with the PTA and/or HHA per regulations.Communication - Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills. Follow-Up Programs - Recommend and ensure continuation of appropriate therapeutic follow-up programs and activities. Develop programs to optimize successful discharge with highest function possible.
