Social Services Assistant

What You'll Do: Assist in the development and implementation of
care plans
that address residents' social, emotional, and psychological needs Support residents and families with
discharge planning, community resources, and emotional support Advocate for residents' rights and work to resolve any concerns or issues Maintain accurate
documentation
and reports in compliance with state and federal regulations Collaborate with the interdisciplinary team to provide a
holistic approach
to resident care Help organize
resident-centered activities and programs
that promote social engagement and well-being What We Offer:

Experience in social services or healthcare preferred, but
we are willing to train the right candidate Compassionate, patient, and
