POSITION SUMMARY
The physician provides primary care in an outpatient clinic setting. Will follow and direct evidenced-based practices as established within the Primary Care service line. Collaborates and coordinates the individual treatment plan with the clinical team as necessary to assure an appropriate continuum of care. Examine patients, take medical histories, prescribe medications; and order, perform, and interpret diagnostic tests. This position will also be responsible for supervising some of the hospitals' mid-level providers.
Minimum Education Requirement :
Completed the approved Doctor of Medicine or Osteopathy residency program in a specialty, such as Family Practice or Internal Medicine/Pediatrics, resulting in board eligibility or certification.
Minimum Experience Requirement :
Must have completed a family medicine or internal medicine/Pediatrics residency. Previous experience in group practice, clinic or hospital setting with a broad scope of family practice or internal medicine/Pediatrics experience is preferred.
Minimum License Requirement :
Current WV Federal Drug Enforcement Agency Certificate. Current West Virginia Medical Licensure. Current CPR certification, ACLS and PALS preferred. Must be fully credentialed according to Grafton City Hospital Medical Staff by-laws.
Responsibilities : Conduct thorough medical evaluations and formulate an accurate diagnosis. Monitor patient progress and adjust treatment plans. Provide health education and counseling for patients. Maintain accurate, up-to-date records.
