Provide support to residents in post-acute facilities as an
LPN Monitor residents’ mental and physical conditions as well as vital signs Facilitate the intake process, documenting medical histories and health issues Assist with medication administration, including verification processes and monitoring effects on residents Maintain accurate medical records and charts for residents Help with immunizations, IVs, and blood draws Ensure a comfortable and safe environment for residents Adhere to residents’ care plans and comply with all regulations Contribute to the maintenance and proper inventory of medical supplies Assist with onboarding and training new hires Qualifications and Skills
Current Connecticut state license to practice as a
Licensed Practical Nurse (LPN) Completion of an approved
Licensing Practical Nurse (LPN)
educational program High school diploma or GED (or equivalent) 6+ months of experience as an
LPN
