As one of Los Angeles County's largest social services agencies, formed in 2016 through a merger of OPCC and Lamp Community, we provide evidence-based solutions to homelessness and domestic violence challenges, including outreach, housing, health care, substance abuse, and life skills programs.
Job Summary
Using harm reduction, housing first, and trauma-informed care, the Case Manager supports clients' housing stability, self-sufficiency, and community integration. Responsibilities include assessing barriers to housing, supporting clients post-housing, and coordinating services to meet individual needs, fostering independent living skills for successful transition from homelessness.
Essential Duties & Responsibilities
- Manage a caseload of approximately 20 high-acuity clients, up to 40 for low acuity.
- Develop relationships to facilitate independence and health maintenance.
- Collaborate with LA County mental health and health services programs.
- Implement client-centered interventions emphasizing harm reduction and trauma-informed care.
- Assist with screening, intake, and orientation processes.
- Conduct biopsychosocial and needs assessments for treatment planning.
- Create quarterly goal plans to improve quality of life.
- Maintain accurate progress notes and data in electronic systems.
- Ensure client documentation compliance and confidentiality.
- Support clients in benefits application and access to healthcare services.
- Coordinate appointments, transportation, and community resources.
- Transport clients in personal vehicle within Los Angeles area.
- Teach and model life skills such as budgeting and health maintenance.
- Communicate effectively with property managers and partners.
- Participate in team meetings, supervision, and training.
- Adjust workload per supervisor guidance.
- Perform additional duties as assigned.
Qualifications
- Preferred Bachelor's degree or two years' experience working with homeless populations, especially with mental illness/substance use issues.
- Valid driver's license, reliable vehicle, insurance, and acceptable driving record.
- Willingness to transport clients in personal vehicle.
- Knowledge of case management preferred.
- Ability to work within harm reduction models with clients facing multiple barriers.
Work Environment
- Combination of field and office work.
- Ability to sit, stand, bend, lift up to 35 lbs, and drive to sites.
- Work in busy, loud, and outdoor environments with exposure to elements.
