Now Hiring: RN Case Manager – Join a Leading Healthcare Team!
Location: Pittsburgh, PA 15230
Full-Time | Competitive Pay | 26Weeks Contract with Potential to Extend
Important: Case managers will cover 5 Skilled Nursing Facilities within 20-30 miles
- Shift: 8 hrs. days Shift, 8a to 5p, (Mon to Fri)
- Scheduled Hours: 40 Hours a Week .
Hourly Compensation: $50.00 - $60.00 Per Hour.
Job Duties:
- Ensure accurate and complete documentation and collaborate with the interdisciplinary care team.
- Assist the provider/team with various care coordination activities.
- Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits.
- Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition.
- Schedule and participate in family conferences, team meetings, and team case presentations.
- Coordination of training, tracking, and compliance with quality measures.
- Assist in creating and conducting services for selected audiences.
- Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
- Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions.
- Assist in managing members during a short-term sub-acute rehabilitation episode.
What We’re Looking For:
- Active and unrestricted RN license for PA State or Multistate.
- Must have case manager or care coordination experience.
- Experience in LTC or SNF is highly preferred
