Provide high-quality nursing care to post-acute residents in the community. Monitor resident conditions and communicate updates to the nursing team. Document vital signs and perform essential nursing procedures. Assist residents with daily living activities, including grooming, feeding, and toileting. Maintain a clean and safe environment in accordance with hygiene standards. Transport residents within the facility and assist with mobility. Engage with residents to foster relationships and provide comfort. Follow and document care plans accurately and adhere to regulations. Facilitate communication among staff, residents, and their families. Qualifications and Skills
Must hold a current Connecticut
Certified Nursing Assistant (CNA)
