Provide nursing care in post-acute care facilities Follow and adhere to care plans for residents Monitor vital signs and report findings to the nursing team Observe residents and report any concerns to supervising staff Assist residents with activities of daily living (ADLs) Maintain a clean and sanitary environment for residents Facilitate communication between residents and healthcare staff Engage with residents for daily companionship Interact with family members to address their concerns Assist in safely moving residents as needed Qualifications and Skills:
