Job Responsibilities Assist residents in ALFs, SNFs, and other post-acute facilities Help with mobility, personal hygiene, feeding, and other daily living activities Monitor residents and report any concerns to the nursing team Check vital signs and inform staff of any abnormalities Maintain cleanliness in resident rooms Assist residents with transfers, such as moving to/from wheelchairs and beds Provide companionship and support to residents and their families Communicate effectively with visitors, family members, and staff Follow and adhere to resident care plans Qualifications and Skills Current New Hampshire
