Under general supervision, the Outreach Coordinator will develop, coordinate, and provide system-wide leadership in delivering patient education programming and outreach services to Ambulatory Care’s patient population and their families. This position will produce written and/or digital instructional material and will gather and maintain data for performance improvement purposes. In addition, this position is responsible for effectively cultivating and managing strategic partnerships with patients, clinic staff, healthcare providers, and community partners through quality management and performance improvement initiatives.

The ideal candidate

will have some technical expertise as it pertains to quality management and/or performance improvement in healthcare administration. The ideal candidate will also have strong communication skills, demonstrate creative thinking, and be able to effectively oversee multiple projects concurrently.

Duties may include, but are not limited to the following:Plans, directs, and coordinates activities designed to foster and maintain patient engagement;Conducts patient outreach utilizing a variety of platforms; such as, text, phone, and mail;Conducts research, analyzes results, and prepares reports and presentations of findings on a variety of special projects;Creates outreach scripts and customized patient education workflows;Proofreads typed materials for correct grammar, spelling, and punctuation;Develops staff protocols and education to promote quality improvement, performance improvement, and patient safety;Develops and implements outreach projects;Utilizes Microsoft Excel to perform data analytics;Maintains a calendar of outreach activities, including arranging consistent outreach communication efforts with patients;Coordinates with clinic staff and healthcare partners to implement and maintain patient incentive programs;Performs other related duties as required.

In addition to the duties listed above, Administrative Assistant III/IV duties may include:Promotes quality/performance improvement activities throughout the organization by developing and presenting educational content at the AC Quality Alliance (ACQA), clinic provider and staff meetings, and other quality meetings;May monitor and supervise the work of others assigned to perform patient outreach.

Typical QualificationsPossession of an Associate Degree in Communication, Sociology, Business, Healthcare Administration or a related field, andOne (1) year of patient outreach, patient advocacy, or healthcare administration experiencePossession of an Associate Degree in Communication, Sociology, Business, Healthcare Administration or a related field, andTwo (2) years of patient outreach, patient advocacy, or healthcare administration experiencePossession of an Associate Degree in Communication, Sociology, Business, Healthcare Administration or a related field, andThree (3) years of patient outreach, patient advocacy, or healthcare administration experience

Substitution: Additional related experience may be substituted for the education on a year for year basis. Additional related education may also be substituted for experience on a year for year basis.
