REQUIREMENTS AND PREFERENCESThe Broward County Board of County Commissioners is seeking qualified candidates for Human Services Coordinator (Community Care Team - CCT) with the Broward Addiction Recovery Division.General DescriptionProvides advanced social case management, counseling, and/or coordination of services in an assigned human services program. Works under general supervision, independently developing work methods and sequences.

Minimum Education and Experience RequirementsRequires a Bachelor's degree from an accredited college or university with major coursework in mental health counseling, marriage and family counseling, counseling education, social work, psychology, gerontology, or other closely related field based upon area of assignment.

Requires one (1) year of direct client social case work/counseling in either an addiction/substance abuse treatment or mental health facility/program, crisis intervention or clinical setting, or in a community resource eligibility center based upon area of assignment.

Special RequirementsDepending upon area of assignment, may be required to work nights/weekends/holidays.

May be required to conduct home and community visits related to consumer or client needs.

Special Certifications and LicensesDepending upon area of assignment, may be required to have a Certified Addiction Counselor (CAC) Certification and a valid Florida Class E Driver's License.

DUTIES AND RESPONSIBILITIESThe functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the job as necessary.Conducts home and community visits related to consumer or client needs as required for area of assignment.Advocates on behalf of clients or consumers and families for services, basic needs, and other related issues.Assists social workers in case conferences and staffing; may serve as team leader.Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications.Conducts activities and monitors the environment of care in a manner to maximize client or consumer and staff safety.Counsels clients or consumers, individually or in groups, and/or their families, to facilitate achieving service plan goals, developing life skills, and/or mitigating inappropriate behavior.Facilitates case consultation for specialized consumer or client situations.Performs needs and comprehensive assessments; develops or updates care plans, service plans, and/or treatment plans with clients or consumers to address their needs and to facilitate referrals to appropriate treatment, service providers, or facilities; engages clients and families in the development of service implementation plans.Provides crisis prevention, intervention, and supportive counseling as needed.Provides follow-up and monitors service delivery and care environments.Provides general information to clients. Coordinates and delivers agency services.Explains the legal process to clients and family members as it pertains to individual cases; coordinates with local law enforcement agencies and the State Attorney's Office regarding the status of cases.Testifies at depositions, hearings, and trials as required.Prepares case records and makes recommendations to supervisor concerning specific cases.Must be able to respond to various locations as scheduled and during on call.Provides resources and connections that affect a person's overall well-being, such as primary physical health care, housing, and social connectedness.Engages the individual in their current setting, such as the crisis stabilization unit (CSU), SMHTF, homeless shelter, detoxification unit, or at their home, etc.Utilizes a standardized level of care tool and assessments to identify service needs and choice of the individual served.Provides frequent contact within the first 30 days of services, which could consist of visits daily to three times per week.Serves as single point of accountability for the coordination of an individual's care with all involved parties.Develops a care plan with the individual based on shared decision-making that emphasizes self-management, recovery and wellness.Coordinates care across systems, to include behavioral and primary health care as well as other services and supports that impact the social determinants of health.Assess the individual for eligibility of Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Veteran's Administration benefits, housing benefits, and public benefits, and assist them in obtaining eligible benefits.For individuals who require medications, ensure linkage to psychiatric services within 7 days of discharge from higher levels of care.Provides rotational 24/7 on-call availability.Performs related work as assigned.

WORK ENVIRONMENTPhysical DemandsPhysical demands refer to the requirements for physical exertion and coordination of limb and body movement.Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.Unavoidable Hazards (Work Environment)

Unavoidable hazards refer to the job conditions that may lead to injury or health hazards even though precautions have been taken.Involves routine and frequent exposure to traffic; moving machinery.
