Codman Square Health Center (CSHC) is a community-based, outpatient health care and multi-service center located in the heart of the Dorchester neighborhood in the City of Boston, Massachusetts.
CSHC was founded in 1979 by a group of community visionaries with a dream to build the best urban community in America. As part of this dream, the founders recognized that though health care begins by alleviating sickness, the journey to a true "culture of health" is achieved through the health of the whole person and community. Serving patients from Dorchester and surrounding Boston neighborhoods, including Roxbury, Mattapan, Hyde Park, and the neighboring municipalities of Randolph and Brockton. CSHC provides comprehensive care for the whole family, including primary, eye, dental, urgent, and behavioral health care, as well as many medical specialties. With more than 115,000 client contacts each year, and a patient base exceeding 24,000, 87% of whom live below 200% of the Federal Poverty Level, and operating revenue nearing $55M. CSHC is the principal option for affordable, high-quality, primary, and preventive care in one of Boston's underserved and most vulnerable communities.
CSHC is a Federally Qualified Health Center and receives federal support through its designation as a Section 330 community health center. CSHC is a licensed health center under Boston
Medical Center which serves as the organization's primary hospital affiliate. In 2016, CSHC was re-recognized as a Level 3 Patient-Centered Medical Home (PCMH) by the NCQA, the highest level of PCMH distinction possible demonstrating CSHC's commitment to innovative and patient-focused care. CSHC also earned PCMH PRIME certification, which indicates high-level PCMH practices that demonstrate the integration of behavioral health into their primary care services.

Position Summary
The CNO has the exciting opportunity to rebuild a department where Nursing functions at the top of their licenses and has an equal say at the table in the delivery of the Integrated Care our patients and community need to achieve exceptional health outcomes. Combining traditional outpatient nursing with Population Health and Care Management techniques, the CNO will be accountable for nursing excellence and have shared accountability for quality outcome measures.
Supervision and Management of Assigned StaffRecruit, hire, and train a diverse nursing leadership. This may include interviewing, selecting, orienting, and training midlevel nursing leaders.Audit, maintain standards and scope of nursing practice; provide direction and support to staff to ensure practice efficiency. communication to nursing staff.Structure opportunities for education, mentoring, and professional growth & development for nursing staff.Promote professional nursing best practice through mentoring, professional development, and collaboration with other disciplines.Establish job guidelines and accountability to ensures that standards and scope of nursing practice are being upheld.Analyze and determine appropriate staff ratios to support provision of services. Take action to ensure adequate support within all clinical departments.Conduct timely performance reviews according to established guidelines.Work with the CHRO to ensure pay equity is examined and managed.Take action to ensure increased nurse and medical assistant satisfaction and engagement are assured.Development and Implementation of Systems that Support the Efficient Delivery of Services
Manage and monitor flow of patient activity.Direct the development and implementation of new programs and services consistent with organizational goals.Develop and implement policies and procedures that improve services and revise them periodically.Ensure client satisfaction by analyzing complaints, concerns, and suggestions and provide sound resolutions.Develop strategies and improvements consistent with the quality improvement program.Ensure effective internal collaboration among medical, nursing, and all staff in the delivery of care.Facilitate cooperative and collaborative external, community, and institutional relationships.Development and Implementation of Systems to Support the Operations of the Health Center Targeted at Improving Provider Efficiency in the Delivery of Services
Develop improved workflow patterns, including live triage and call center triage nursing functions.Develop scheduling protocols that support adequate coverage, patient access, practice efficiency, staffing support, and use of space.Evaluate the volume budget for nursing visits.Accountable for achieving or exceeding service line targets for nurse visits and develop innovative ways to capture nursing billing opportunities.Evaluate and monitor schedule protocols against established benchmarks.Assess practice capacity and capabilities for managing new contracts, grants programs, or services.Develop reports and supporting data to ensure the performance standards for the health center are being met or exceeded in conjunction with the Chief Medical Officer.Work with the CFO to review reimbursement procedures and systems (i.e., coding/encounters) that support more efficient fiscal management.Accountable for nursing department P & L (or statement of operations) performance to budgetDevelopment and Implementation of Systems to Support the Operations of the Health Center Targeted at Achieving/ exceeding Accountable Care goals of Improved Health Outcomes
Oversee both Care management and Population Health nurse staffing.Work with the CIO to develop useful and timely reports for the clinical teams to manage patient panels.Design and implement nursing programs to engage patients in their care.Support and implement dyad and triad team structure for panel management with Medical and Operations leadership and staff.Engage with other local, regional, and national CNO to share best practices and address critical issues.Provision of Leadership to Assigned Departments
Prepare and monitor budget.Determine capital, supply, equipment, and operating needs for assigned departments. Strengthen the management of ordering and inventorying medical supplies and medication.Participate in meetings and on committees in discussion of goals, visions, and strategic plans for clinical services.Provide effective communication to staff on vision, goals, and procedures. Also acts as a conduit for communication from staff to Executive Management.Lead development of the department's initiatives designed to foster team development and maintain an outstanding customer service culture.Ensure the department meets all regulatory requirements (e.g., JCAHO, DPH, etc.).Oversight of compliance for grant funded programs.Oversight of the infection control preventions and the infection control plan for the organization. This includes developing and implementing infection control policies and procedures, conducting surveillance activities, and educating staff and patients on infection control practices.Qualifications
Competencies, Skills, and Attributes:
Prior supervision of Nurse Clinical Leaders for Family Medicine, Internal Medicine, Pediatrics, Urgent Care, Complex Care Management and Population Health. Experienced with hiring, disciplining staff. Champion timely performance reviews for all nurses and medical assistants.Demonstrated experience using reporting tools to leverage data to improve workflow, SOP's, and quality of care performance.Represent nursing function in Executive TeamQualifications and Skills:
Bachelor of Science degree in Nursing is required. Master of Science in Nursing is preferred. Relevant work experience will be considered.Current license to practice professional nursing-RN in the Commonwealth of Massachusetts required.Previous leadership experience within a diverse community health center setting preferred.Clinical Experience and strong knowledge of Epic electronic health record.Must be able to act cooperatively and collaboratively as a member of an interdisciplinary team. Require strong interpersonal, organizational, and communication skills. Must have demonstrated commitment to the principals of collaboration, teamwork, and innovation.Strong working knowledge of Excel.Grants ManagementPhysical Requirements:
