In this role, you will lead the medical aspects of team-based care for individuals with serious mental illness, conducting whole-person assessments while collaborating closely with community primary care and behavioral health providers.Responsibilities:
Train and advise Medical Assistants and Peer Recovery Specialists on behavioral, clinical, and social health prioritiesSupport care coordination effortsLead care and disease management initiatives, identifying actionable care gapsCommunicate and coordinate with individuals' care providers, primarily at CMHCs, FQHCs, and hospitalsProvide direct patient care when unscheduled needs cannot be met by existing community providersPerform other duties, as neededQualifications:
VA Nurse Practitioner (NP) and/or Physician Assistant (PA) licenseDesired Skills:
2+ years of experience as a Nurse Practitioner (NP) and/or Physician Assistant (PA)Experience working with behavioral health patients
