Hospital is seeking a Family Practice Physician that specializes in Sports Medicine.The following is a list of the compensation and benefits to be included in an employment contract:Term: Twenty-Four (24) Months w/ Annual RenewalsBase Compensation: $212,000 Base SalaryProductivity Package: WRVU’s > 3,968 Produced in the Family Practice Office, Paid at $30.00Health Insurance: Covered by the Practice for Participation in H.S.A. PlanDisability Insurance: $6,000 per Year for Documented ExpensesDues/Membership/CME Fees: $5,000 per year for Documented ExpensesMalpractice Liability: Covered for Active Physicians; Tail Prorated Based on Length of ServicePersonal Leave: 4 weeks (20 days) Paid Time off for Sick/Vacation TimeCME Leave: 2 weeks (10 days) Paid Time Off for Educational PurposesPension Plan: 401(k) With a 2.5% Match capped at IRS LimitWork Schedule Expectation: 40% Student/Athlete Health; 60% Family Practice
