Job Description:- Provide case management services in an acute care hospital setting.- Manage patient discharge planning processes.- Utilize documentation systems such as Meditech, Midas, and Careport.- Coordinate with unit secretaries and techs to ensure efficient unit operations.- Adhere to business casual dress code with a white lab coat.- Work closely with a team of experienced case managers.- Participate in ongoing training and development opportunities.- Contribute to high patient experience standards.
