Central Ohio Primary Care is seeking a Registered Dietitian Nutritionist (RDN) with our Diabetes Management Group located in Westerville, OH. The Registered Dietitian will be working primarily with patients who have a diabetes diagnosis. The Dietitian will provide medical nutrition therapy to improve the patient's health outcomes and help meet their goals. This position will work alongside the RN Diabetes Educator. This will be a full-time position working first shift hours Monday- Friday.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Provide medical nutrition therapy for COPC diabetes patients as referred from the primary care provider or Endocrinologist.Offer individual counseling as well as group classes to provide nutrition education on diabetes to prevent complications with lifestyle related modifications.Use motivational interviewing strategies to encourage positive behavior change. Work with the patient to create individualized goals and monitor progress through follow up visits.Communicate regularly with the referring providers to discuss patient's progress. Coordinate care and appointments with additional health care providers.Support program standards and ensure compliance with ADCES requirements.Document all progress notes and outreach attempts in the Electronic Health Record.Manage a rotating schedule at several endocrinology and medical practices including the local travel between locations.Contact and schedule nutrition referrals as neededServe as a preceptor to dietetic interns from accredited universities.Contribute to company-wide health education through office engagement, wellness blogs, and employee seminars as needed.QUALIFICATIONS:
Education, Licensures & Certifications
Required: Bachelors Degree in Nutrition or DieteticsRequired: Registered Dietitian with the Commission on Dietic RegistrationRequired: Licensed Dietitian with the Ohio State Medical AssociationRequired: Active Ohio driver's license with the ability to travel throughout COPC's service areaKnowledge, Skills & Abilities
Experience working with a diabetes population, preferred.Experience working in an outpatient setting, preferred.Ability to multi-task, work independent, and creatively solve problemsDetail oriented with strong analytical, organization and time management skillsStrong customer service skills with ability to work well with physicians, APPs, clinical staff and administrative staff.Team-oriented and work well in a collaborative environmentExcellent written and verbal communication skills, especially in stressful situationComputer proficiency, especially with Microsoft office productsElectronic Medical Record proficiencyStrong organizational skills required
