Outpatient Social Worker
POSITION DESCRIPTION:
The Outpatient Social Worker provides psychosocial support and counseling to individuals and families in an outpatient setting. This role involves assessing client needs, developing treatment plans, and connecting clients to resources that support their mental, emotional, and social well-being. While primarily serving dialysis and oncology, the outpatient social worker may support inpatient services as needed.
Supervisory Responsibilities:

Duties/Responsibilities:
Conduct comprehensive assessments of clients psychosocial needs, strengths, and challenges to inform treatment planning.Collaborate with clients to develop personalized treatment plans, setting achievable goals and monitoring progress.Identify and connect clients with community resources, including housing assistance, financial support, healthcare services, and educational programs.Assess and respond to crisis situations, providing immediate support and intervention as necessary.Advocate for clients needs within the healthcare system and community, ensuring they receive appropriate services and support.Work closely with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to coordinate care and ensure holistic treatment for clients.Maintain accurate and timely records of client interactions, treatment plans, and progress notes in compliance with organizational policies and regulations.Provide education to clients and their families about mental health, coping strategies, and available resources to enhance overall well-being.Complies with all safety and regulatory requirements in the care of oncology patients.Other duties as assigned.

Education/Experience:
Masters degree in Social Work (MSW) from an accredited program.Previous experience in an outpatient setting, particularly with populations facing mental health challenges or chronic illnesses, is preferred.
Licenses/Certifications:
Current state licensure (e.g., LCSW, LMSW) or eligibility for licensure.Basic Life Support (BLS) certification required
