Job Title: ACM Coordinator I
Department: Health Services - Ambulatory Care Management (ACM)
About the Role:
We are currently seeking a highly motivated Case Management Coordinator who will assist the Case Managers with day-to-day care management responsibilities. This role will report to an ACM Supervisor and enable us to continue to scale in the healthcare industry.
What You'll Do:Responsible for member/provider outreach and notification (telephonic and/or written) utilizing department templatesResponsible for assisting ACM nurses with member assessment and communication, which includes but is not limited to eligibility verification, requesting medical records, creating cases, and documenting care activityParticipate in all health plan and department meetings and training as applicableAssist with health plan audit preparationMeet standard benchmarks with performance objectivesProvide ongoing support and mentorship to other team members as needed.Answer inbound phone queue calls timely to assist members and/or providers.Assist in delegated projects and assignmentsPerform other duties, projects, and actions as assignedQualifications:
High school diploma OR GEDExperience using Microsoft applications such as Word, Excel, and OutlookAbility to travel to our Monterey Park, CA officeExperience working in healthcare setting preferredHighly preferred

Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 1600 Corporate Center Dr, Monterey Park.The total compensation target pay range for this role is: $20-25 per hour. The salary range represents our national target range for this role.
