Starting Pay Rate: 24.03
I) Job PurposeTo work closely with clients to build their self-efficacy base on a Housing First model utilizing relation theory and stages of change concepts, equipping clients with skills to sustain sobriety and recovery from addiction, trauma, mental health issues and other concerns. The goal is to increase client's self-sufficiency and maintain their housing arrangements. Specialist will work with all referred High Priority Directions Home clients and their housing case managers to develop a treatment plan based on individual needs. Follow-ups will be provided as needed to care for the client and to communicate with the referring case manager.
II) Essential Functions
Provides individual counseling, group therapy, screening, information, education, and referrals to High Priority housed clientsWork with each client to develop a strategy of how to best address their individual concerns and goals to maintain their housing stability.Assist clients in utilizing service providers by transporting client to appointments, role-playing, problem solving and being a source of encouragement and inspirationWeekly check-ups with treatment or mental health provider staff to discuss client's progress.Provide individual counseling with clients when needed and collaborate with client and housing case manager to develop a treatment plan to document appropriate goal system.Work with group facilitator to provide Tennant Support Service groups to all Directions Home housed clients.Make referrals to supportive groups and encourage clients to attend.Facilitate groups on an as needed basis in the absence of primary facilitator.Maintain a sense of connectedness to each clients' growth and development by continually evaluating the progression of growth and developmentDocument specific actions the client is taking that are progressively moving the client through the stage of change and discuss those accomplishments regular with the client.Maintain regular and frequent communication with each client to reinforce your connectednessCollect and prepare data for computer entry, evaluation, and reporting program measuresWork closely with Program Manager to effectively and efficiently achieve program measuresPerformance standards are performed as applicable with MHMR's

III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and AbilitiesKnowledge of MHMRTC and Addiction Services Operating ProceduresKnowledge of DSHS/CARF standardsKnowledge of the treatment of substance abuse.Ability to provide counseling in substance abuse and mental health in individual and group sessions.Ability to communicate effectively in both verbal and written forms.Ability to establish and maintain positive working relationships.Specific knowledge of family, vocational, legal, cultural, and other sociological problems associated with substance abuse.Ability to conduct screenings, referrals, intakes, assessments and unit orientations.Ability to provide crisis intervention when needed.
IV) Internal & External Customer ServiceExtensive internal and external contact is required. The employee will demonstrate advanced written and verbal skills while maintaining a positive image
B) Must demonstrate compassion, trauma-informed care, and safety practices including suicide safe care
V) TravelInter-agency travel is required. Employee is responsible for making their own accommodations for inter-agency travel
