Description

Greater Nashua Mental Health is looking for a full-time Team Coordinator to join our Older Adult Services team! The Team Coordinator clinically and administratively manages the Older Adult Services team. Responsible for the OAS team's performance on KPI's, including productivity expectations and paperwork completion. Oversees the professional development of direct reports. Expands Evidence Based Clinical Practices within the department, spearheads, and champions quality initiatives, and refines clinical practice guidelines, processes, and procedures within the department. Responsible for selecting and training staff, as well as understanding the philosophy behind Referral, Education, Assistance, and Prevention Program (REAP), and other clinical practices. Implements these practices within the treatment team, while supporting the team in the areas of clinical, administrative, and professional activities.

Duties & Responsibilities:

Provides weekly one (1) hour individual supervision to master's level therapists and coordinator of community-based services.Prepares for and attends weekly team meeting (1 per week), staff meeting (1 per week), therapists' meeting (1 per week), Community Support Services (CSS) department meeting (1 every other month), Elder WRAP (Wellness Recovery Action Plan) Meeting (1 per month) and supervision (1 per week). Completes all documentation of the supervision sessions in a timely manner.Provides ad hoc support to staff for crisis management and guidance. Also provides supplemental counseling to members of clients' support systems (e.g., family members).Provides at least eight (8) hours per week of psychotherapy for individuals, groups, and/or families, and case management services.Completes treatment plans, Adult Needs and Strength Assessments (ANSA) and Case Management Care Plans (CMCP) by the required due date listed in EMR (Electronic Medical Record) and supports and monitors staff doing the same.Completes all Quarterly Reviews within a two (2) week window at the end of each review period and completes all other documentation within three (3) business days (with the first day being the dates of service/completion) and supports and monitors staff doing the same.Provides clinical input and formulation of Older Adult Services Treatment Plans which identify specific problems to be addressed, target goals related to problems, and clear and measurable objectives with clear time frames that are attached to specific services to be provided.Reviews Treatment Plans to ensure they are client centered, completed with client's input and have SMART goals.Oversees the development of relevant therapy groups to OAS clients that consider the needs of group members.Develops or oversees the development of various protocols to support OAS services, e.g., intake, discharge, referral process, etc., in collaboration with the Director of Quality and Corporate Compliance.Communicates, collaborates, and consults with other staff and agencies.Reviews cases referred to the OAS Teams to determine appropriateness of admission.Coordinates referrals and reviews results of additional assessments as appropriate.Provides education and consultation to OAS Team, agency staff, external agencies and the community at large as required by the Director of Adult Services or the Senior Leadership Team.Locates, develops, and uses internal and external resources for consumers, as needed.Maintains knowledge of department and agency policies and procedures and has knowledge of other resources and services provided within the agency and community.Participates in training/staff development activities consistent with in-service training schedules and staff development plans.Completes all required administrative assignments including monitoring schedules, data reporting, and QA Activities timely.Ensures OAS provides REAP services including direct and indirect Support Services to older adults and their families in their homes, in the community and at times in the office.Ensures OAS has a designated REAP provider and actively participates in REAP trainings in addition to required agency trainings. This includes annual REAP training and training related to REAP.Ensures REAP provider is trained to complete accurate needs assessments and document all REAP activity promptly in the REAP reporting website.Provide internship and/or licensure supervision.Performs other duties or special projects as required or as assigned.Requirements

Qualifications:

Master's Degree in Psychology, Social Work, Human Services or related fieldLicensed or license eligible in NH; MLADC certification preferable3 to 5 years related work experienceExperience working with people diagnosed with mental health disorders, co-occurring severe mental illness, and substance use disorders.Ability to lead and guide staff. Flexible, willing to learn, and anticipates the needs of self and staff.Familiarity with community resources.Ability to coach / teach effectively.Demonstrated skills in crisis intervention.Strong collaborating skills and an ability to work effectively independently and as part of a team.Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.Excellent communication and interpersonal skills.Ability to multitask and be flexible with workload and assignments.Ability to problem solve and exercise judgment.Excellent organizational skillsKnowledge of Microsoft Office products (Email, Word, EXCEL).
