Clinical Director of Value Based Care reports to the Chief Medical Officer
and works collaboratively with operational and clinical leaders and colleagues to advance population health management and value-based care strategies. S/he works within a matrixed organization to hold clinical and administrative teams accountable for the implementation of services, and achievement of goals, including assessment and adaptation of clinical model design, measurement of performance, and influencing care quality.Essential Duties & ResponsibilitiesWe seek clinicians (MD, DO, or DNP) of diverse backgrounds who are excited to engage in clinical, educational, and teaching activities.Assisting in offering constructive feedback to providers to improve performance.Helping providers master data-driven tools and practices required to keep patients well and out of the hospital.Reviewing quality dashboards with providers and implementing initiatives to improve quality outcomes.Providing managed care perspectives to providers as they manage patient care.Understanding and communicating how care provided impacts provider performance/compensation.May assist in developing a pipeline of internal clinical leaders skilled in problem solving, communication, conflict resolution, value-based care delivery, and collaboration with leadership.The Clinical Director of Value Based Care primarily focuses on the management of care in a territory or geography, will have input into a variety of organizational-wide projects, and will join committees for clinical programs to support the enhancement of the care model, including:Medical ManagementCare ManagementQuality ImprovementHEDISUtilization ManagementNetwork ManagementHealth Plan ManagementDocumentation/Risk AdjustmentPharmacy utilization/GDRClinical Compliance & Policy DevelopmentThis includes representing Cano Health to external partners (e.g., payers, providers) and creating relationships and partnerships that support our mission and our economics.Additional Duties & ResponsibilitiesPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the associate for this job. Duties, responsibilities, and activities may change at any time with or without notice.Education & ExperienceMedical degreeCurrent National Board Certification or eligibility in Family Medicine, Internal MedicineDNP degree with certification in primary care/GeriatricsExperience in Medicare Advantage Risk and/or Value Based Care ManagementExperience leading and successfully managing providersKnowledge, Skills & ProficienciesEither M.D. / D.O. / DNP graduates.Physicians should be board certified in Internal Medicine or Family Medicine; DNPs should be certified in a primary care domain.Fellowship training in Geriatrics and/or other professional degrees (e.g., M.B.A., J.D., M.P.H.) welcome but certainly not required.Minimum of 5 years of experience in outpatient practice.Minimum of 3 years of experience in a physician (Clinician) management role or role as medical or clinical director, managing a medical group of 5 or more providers.Extensive experience in clinical leadership roles, leading and coaching physicians to be the best they can be for their patients and their colleagues.Experience with managed care and/or value-based practice and familiarity with payer-provider collaboration.Experience using a metrics-driven approach to analyze cost, quality, and satisfaction data to drive clinical strategy and program redesign.Excited by developing and implementing new processes.Self-aware and confident in their leadership skills and eager to share those skills with a fast-growing, passionate team on the leading edge of healthcare innovation.Understand the basics of managed care and recognize that population health focuses on smart allocation of scarce resources leading to high value care (better care, lower cost).Ability to remain calm in stressful situations.Commitment to Cano Health’s patient population.Comfort with ambiguity and a strong desire for problem solving.US work authorization.Physical RequirementsThis position works under usual office conditions. The associate is required to work at a personal computer as well as be on the phone for extended periods of time. Must be able to stand, sit, walk, and occasionally climb. The incumbent must be able to work extended and flexible hours and weekends as needed. Physical demands include the ability to lift up to 50 lbs. The physical demands described here are representative of those that must be met by an associate to successfully perform the essential functions of the job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Work ConditionsMust be able to perform essential functions such as typing, standing, sitting, stooping, and occasionally climbing.Travel RequirementsAmount of Expected Travel : Yes, 25-50%.
